Summary of Duties: 
As part of the treatment team providing services to persons with severe and persistent mental illnesses, a case coordinator establishes a therapeutic alliance with the client, works with the client to complete assessments and tools to determine level of care and need, assists the client in learning life skills that will enhance their quality of life and monitors psychiatric social functioning to ensure safety in the community. This can include but not limited to: (a) meeting with the client on a scheduled basis to assess current functioning and to teach skills, (b) providing assistance on behalf of the client, as needed in linkage with entitlements, service agencies and the medical community and (c) jointly developing a treatment plan and regularly assessing progress in achieving those goals. If Case Coordinator is Master’s degreed, participation in the Access Center Service is required.
